Laser depilation: permanent epilation
I - Introduction
The laser technology revolutionized these last years depilation thanks to selective photo-thermolysis causing a thermo-coagulation of the hair bulb: pushes back will not take place more, the hair is definitively destroyed.
The technological challenge is to allow this thermo-coagulation of the hair bulb without inducing unfavourable effects on the adjacent skin.
That is now possible thanks to the lasers of new generation which associate an optimal wavelength, an effective system of cooling and an end règlage of the various parameters.
II - Physiology of the hair
The hair develops according to a cycle:c' is the pilar cycle.
The phase anagene is the phase of growth leading to the formation of a new hair.
At one moment T, some is the treated body zone, 20 % to 30 % of the hairs are in phase anagene: only 20 % to 30 % of the hairs are thus sensitive to each meeting with the laser.
Indeed, it is at the time of the phase anagene of the pilar cycle that the bulb is charged with pigment of mélanine allowing the effectiveness of the laser
During the phase catagene and telogene
respectively phase of rest and fall, the laser is ineffective.
III - Laser: operating mode
The lasers are sources of very intense, monochromatic and coherent light (emitted over the same wavelength).
For laser depilation, are used, under control and medical responsibility for the medical lasers of class IV
The laser with technology diode emits a powerful luminous flash of 800nm.
This light is mainly absorbed by the mélanine.
The mélanine is the dark pigment contained in the hair and the cells located at its base.
This pigment is also present at the level of the skin.
The settings must optimize the absorption of the light by the mélanine of the hair at the expense of the mélanine and the cutaneous oxyhaemoglobine.
IV - Our technological choice
For technical reasons, wavelength far away from the red, system of Parmentier cooling, paramètrage règlable easily, we chose to work with the laser Lumenis diode light sheer, 1600 Watt, 800nm, 10-40 J/cm2 which is recognized, by the profession, like the best instrument.
The wearing of glasses is obligatory as much for the patient that for the operator.
V - Consultation of information - parametrage
The expert has excellent clinical signs allowing
to prejudge effectiveness of the treatment.
The determination of the phototype or the rate of mélanine
contained in the skin and its appendices is a fundamental therapeutic
precondition.
A) Classification of Fitzpatrick
Phototype I : Very white skin, very great sensitivity to the sun burns but does not bronze, not mélanine. Hairs dark or clear.
Phototype II : great sensitivity to the sun. Always burn bronzes with difficulty. Very few mélanine. Dark hairs or clear hairs.
Phototype III : sensitive to the sun burns but finishes by bronzing, few mélanine, (chatains clear eyes. Dark hairs or hair lights
Phototype IV : quantity of mélanine average, not very sensitive to the sun bronzes without burning, average skin standard European (brown dark eyes, matt skin). Dark or clear hairs.
Phototype V : quantity of mélanine important, not very sensitive to the sun, matt skin (Asian chechmate, mongrel méditéranéens), dark hairs. Prudence at the time of the treatment.
Photype VI : great quantity of mélanine, nonsensitive to the sun, black skin, dark hairs. Extreme prudence at the time of the treatment.
Bronzing makes the depilation laser less effective because
of the character surface of the pigments of mélanine in the event of
bronzed skin.
The use of the laser is indicated against in the event of: pregnancy, of multiple beauty spots, catch of Roaccutane, catch of photosensitizing drugs.
The sun, UV, car-bronzing, activators of bronzing are disadvised during the treatment.
A white Skin and black hairs constitute the ideal conditions for the depilation-laser.
In the event of bronzed or sunk skin a paramètrage précis(fluence, duration of pulsates, width of pulsates, compression) must be carried out
VI - Laser depilation : modalitEs practical and rate/rhythm preparation of the meetings
At a "laser" meeting the doctor will practise a sweeping of the zone to be treated.
An effective shooting will cause to create a small red cutaneous mark: the root of the hair "was roasted".
The meetings had 10 minutes (armpits) at one hour (whole legs) according to the zones to treat.
The treated person picotement feels one at the time of the meeting, a simple heating afterwards.
A érythémateuse zone is present during 24 at 48 H after the meeting.
The erythema disappears in less than 48 hours.
Seldom, some croûtelles can occur.
An anaesthetic cream can be applied approximately 1h to 1h and half before the meetings.
The cream is withdrawn right before the treatment.
It is important that the zone to be depilated is shaven before the meeting (razor or depilating cream 1 to 2 days front) in order to reduce the épidermique heating.
Pulling up with wax or the grip is contra-indicated before the meetings: that would withdraw the targets.
The felt picotements vary according to the density in mélanine (shirt, internal face of the thighs).
The rate/rhythm of the meetings vary according to the zones to treat.
Thus, for the legs, of the spaced meetings (2 to 4 months during one year is 5 meetings) are recommended
To the level of the upper lip, brought closer meetings are recommended: 4 to 6 weeks during 6 to 8 months is 7 to 8 meetings.
Laser depilation is known as of long duration or permanent
according to the zones.
On the level of the legs, the hairs naturally tend to rarefy with the age. Thus laser depilation on this level is final.
Other zones (chin, upper lip) densifiant itself with the age require a treatment of annual or biannual maintenance.
Pushes back then is weaker and slower.
VII - Results
A reduction from 20 to 30 % of the pilar population after each meeting is observed.
Moreover, the time of pushes back for the other hairs increases, the pilar diameter decreases and the colouring of pushes back pilar changes (clarification).
VIII - Price
The price of the treatments of depilation laseR is calculated on the basis of of an average
pilosity and an average body surface.
The modifications of these parameters, and in particular of the
density of pilosity can involve an adjustment of the price, explained
in the estimate given before each treatment.
No payment in advance is practised.
Laser depilation being a medical act, the payment can be carried
out only after the meeting.
Except preliminary agreement, the care of laser depilation is
not dealt with by the Social security.
Fixed prices are possible in the event of multiple zones to
depilate. : they are calculated at the time of the first consultation (free).
